A group of elderly gentleman were sitting under an aging tree, seeking refuge from a weathered pine in the middle of the day, chatting idly with each other in grace and stillness. This is in contrast with the busy modern facilities and buses in the background where life seems to always be in a constant rush. Aging in place and in an ever expanding community means finding our own rhythm in keeping with our priorities, just like the pine which seems to have find a place for itself, anchoring with it the memories, continuity and it's own existence in that place.

With steady hands and quiet determination, he waters his garden—each drop nurturing not just the plants, but his strength, independence, and purpose. A gentle reminder that even in later years, life continues to grow beautifully.

Healing Through Play, Laughter And Connection
This photograph shows two elderly patients who had both lost their ability to walk. When they were first admitted, they were bed neighbours, lying side by side — but they never spoke, never smiled. Each of them was going through their own struggle, quiet, withdrawn, and honestly, quite lost.
Over time, through rehabilitation, something started to change. The silence slowly broke. In this moment, they’re no longer just patients defined by their condition — they’re laughing, playing, and sharing something together. A simple game turned into a connection.
As part of the rehab team, moments like this hit differently. It’s not just about getting someone to stand or walk again. It’s about helping them feel like themselves again — to regain confidence, dignity, and a sense of purpose.
For many older adults, illness takes away more than just physical ability. It affects who they are and how they see their place in the world. Rehabilitation helps rebuild that — not just function, but meaning and belonging.
To me, this is what ageing in place really looks like. Not just staying somewhere familiar, but being supported, feeling connected, and not going through it alone.

In acute geriatric care, maintaining independence in basic activities of daily living is a powerful marker of quality of life. This moment captures resilience beyond age, reminding us that functional engagement remains possible — and essential — even in the oldest-old.

Bridging Care through Home Medication Review
This photograph captures a home visit session where my colleague and I conducted a medication reconciliation interview with the patient’s caregiver, her husband. During the visit, we reviewed the patient’s prescriptions, discussed medication administration practices at home, and assisted in coordinating follow-up appointments across multiple facilities.
This engagement highlights the crucial role of pharmacists in identifying and resolving drug-related problems, including potential drug interactions, improper storage, expired medications, and unnecessary therapies. More importantly, it empowers both the patient and caregiver to better understand the medications and overall treatment plan. Through attentive listening and active communication, pharmacists can bridge gaps in understanding, improve medication adherence, and ultimately optimise therapeutic outcomes and quality of life.
